Out of 288 Hong Kong cancer patients, 92.3% include themselves in decision-making, 71% prefer joint decision-making: with family, with doctor, with doctor plus family, with family minus doctor, and with doctor minus family. <5% want decision-making by “doctor-alone” and <1% desire decision-making by “family-alone”. Harmony, communication and responsibility are reasons for family participation. Most patients prefer “specialist” for information, followed by “family”, “friends”, and “GP”. Trust in doctors and prospects for controlling/curing disease are important factors in decision-making. Patients want to (...) participate and be listened to rather than make decisions. Doctors should not disclose information to family without patients’ permission, nor withhold information from patient [End Page 109] on family’s request. Only 25% of patients have discussed post-competent treatment with others and 7% have heard of Advance Directive ; 24% refuse to discuss it. After AD has been explained to them, 66% remain reluctant to sign one, preferring to leave oral instructions or appoint family as proxy decision-makers. The family’s decision-making role increases when patients become incompetent. The top reason for signing AD is to receive preferred post-competent treatments, and for not signing it, difficulty with making a prospective decision. Although considerable research indicates that mental energy is an important factor in many domains, including athletic performance (Cook & Davis, 2006), athletic mental energy (AME) has never been conceptualized and measured. Therefore, the aim of this study was to conceptualize and develop a reliable and valid instrument to assess athletic mental energy. In Study 1, a focus group interview established the initial framework of athletic mental energy. Study 2 used a survey to collect athletes’ experiences of athletic mental energy and (...) develop a scale draft titled “Athletic Mental Energy Scale (AMES).” In Study 3, we examined the psychometric properties and the underlying structure of AMES via item analysis, internal consistency, and exploratory factor analysis (EFA). In Study 4, we used confirmatory factor analysis (CFA) to examine AMES’s factorial validity; and examined concurrent and discriminant validity by examining correlations with athletes’ life stress, positive state of mind, and burnout. In study 5, we examined the measurement invariance of the 6-factor, 18-item AMES with Taiwanese and Malaysian samples. Study 6 examined the predictive validity by comparing AMES scores of successful and unsuccessful martial artists. Across these phases, results showed a 6-factor, 18-item AMES had adequate content validity, factorial structure, nomological validity, discriminant validity, predictive validity, measurement invariance, and reliability. We suggest future studies may use AMES to examine its relationships with athletes’ cognition, affect, and performance. The application of AMES in sport psychology was also discussed. (shrink)

Scholars and leaders have assumed that prosocial motivation is the primary driver for social enterprises (SEs) to do good. Despite the significance of prosocial motivation, we argue that it may not be the single driver for SE motivation. A dual-motivations approach based on insights from motivation research is proposed to examine how different types of SE motivation can act together to influence the outcomes of SEs. Empirical findings based on survey data from Hong Kong demonstrate that dual motivations—prosocial-intrinsic and prosocial-extrinsic (...) motivations—explain the outcomes of SEs beyond what the direct effects of prosocial motivation can explain separately. Based on moderation analyses, intrinsic motivation enhances the positive relationships between prosocial motivation and business and social outcomes, whereas extrinsic motivation undermines the relationship related to business outcomes. Implications for research on social entrepreneurship and business ethics are discussed. (shrink)

We propose a new picture of nature in which there are only two fundamental universal constantsè ē (≡e/c) andh(≡ħ/c). Our theory is developed within the framework of a new four-dimensional symmetry which is constructed on the basis of the Poincaré-Einstein principle of relativity for the laws of physics and the Newtonian concept of time. We obtain a new space-light transformation law, a velocity-addition law, and so on. In this symmetry scheme, the speed of light is constant and is completely relative. (...) The new theory is logically self-consistent, and it moreover is in agreement with all previously established experimental facts, such as the “lifetime dilatation” of unstable particles, the Michelson-Morley experiment, etc. There is a difference relative to the usual theory, though, in that our theory predicts a new law for the Dopplerfrequency shift, which can be tested experimentally by measuring the second-order frequency shift. Background Medical Assistance in Dying in Canada came into effect in 2016 with the passing of Bill C-14. As patient interest and requests for MAID continue to evolve in Canada, it is important to understand the attitudes of future providers and the factors that may influence their participation. Attitudes towards physician hastened death in general and the specific provision of MAID are unknown among Canadian residents. This study examined residents’ attitudes towards PHD and MAID, and identified factors that may influence (...) their decision to participate in PHD and provide MAID. Methods A cross-sectional survey was adapted from prior established surveys on MAID to reflect the Canadian setting. All Canadian family medicine programs were invited to participate. The survey was distributed between December 2016 and April 2017. Analysis of the results included descriptive statistics to characterize the survey participants and multivariable logistic regressions to identify factors that may influence residents’ attitudes towards PHD and MAID. Results Overall, 247 residents from 6 family medicine training programs in Canada participated. While residents were most willing to participate in treatment withdrawal, active participation in PHD and MAID by prescription of a lethal drug and lethal injection were less acceptable. Logistic regressions identified religion as a consistent and significant factor impacting residents’ willingness to participate in PHD and MAID. Residents who were not strictly practicing a religion were more likely to be willing to participate in PHD and MAID. Increased clinical exposure to death and dying crudely correlated with increased willingness to participate in PHD and MAID, but when examined in multivariable models, only a few activities had a statistically significant association. Other significant factors included the residents’ sex and location of training. Conclusions Residents are hesitant to provide MAID themselves, with religious faith being a major factor impacting their decision. (shrink)

Ribeiro’s Sextus, Montaigne, Hume: Pyrrhonizers is a charming and quirky investigation of his three titular skeptics. It is perhaps best understood as a skeptical investigation of skepticism. By that I mean that, like a good Pyrrhonist, Ribeiro explains how (...) things appear to him without claiming to lay down a doctrine or dogma. His topic is how and why skepticism still matters (7). And he suggests that those who are moved by skeptical arguments can learn intellectual modesty from them and thereby achieve a degree of philosophical “calmness of soul” (141).Ribeiro approaches his three figures by taking the cogency of their skeptical arguments for granted. There is little investigation of why these philosophers call various beliefs into question, and his concern is rather what remains once the skeptical doubts are taken on board. Because what he calls “Pyrrhonizing” philosophers reveal that large swaths of our beliefs lack epistemic merit, they also show us how we are not fully rational creatures in our everyday lives. In the case of Sextus, Ribeiro’s focus is on the famous question of whether skeptics can live their skepticism once they have actually suspended all belief. With Sextus, as with Montaigne and Hume, Ribeiro opts for the more radical of the interpretive options; he is not persuaded by Michael Frede’s suggestion that Pyrrhonists restrict their suspension only to abstruse matters, not the everyday (51). But even if we follow the “four-fold way” (20–23) that Sextus offers us as guides for life—the directives of nature, feelings, custom, and expertise—Ribeiro still thinks we will inevitably fall back into belief. He concludes that the original Pyrrhonists should be read aspirationally: insofar as we develop our capacities [End Page 517] to argue on both sides of every question and thus are able to suspend judgment, to that extent we will achieve tranquility. In the case of Montaigne, Ribeiro focuses on his Christian fideism. Once he recognizes the incapacity of reason to resolve intellectual problems, he falls back on faith. And Ribeiro argues that the Catholic flavor of this faith is simply a reflection of Montaigne’s surroundings. In a different community, he could just as easily have embraced a different faith. The heart of the book concerns Hume, whom Ribeiro discusses in three chapters, as opposed to the one each devoted to Sextus and Montaigne. In keeping with his interpretive preferences, Ribeiro’s Hume is a radical skeptic, though he acknowledges the significant yet “utterly perplexing” (95n22) naturalist strains that are also present. Ribeiro takes the resulting tension to be insoluble (98), though I was not convinced. Consider that, in both the Treatise and the Enquiry, Hume uses causal reasoning to investigate causal reasoning. He learns that our ideas are caused by prior impressions; that repeated experience of conjoint events causes us to associate our ideas of similar events; and so on. In the Treatise, he concludes the argument that association is at the root of such causal reasoning by presenting a set of rules, a “logic,” for it. The argument is not as such skeptical, though when Hume “recasts” it in the first Enquiry, he does so in terms of “sceptical doubts” and a “sceptical solution.” In the Treatise, “total” and “extravagant” skepticism come into play only as “systems of philosophy” that Hume uses to shed light on his own core commitments. The problem, I think, is that Ribeiro tends to read Hume a little too literally, as if he is himself falling prey to the skeptical upheaval of the conclusion to the first book of the Treatise, or the final section of the first Enquiry. But I would suggest that Hume is rather deploying the rhetoric of skepticism in these discussions, using it for his ultimate philosophical ends (a point that is especially clear in Ribeiro’s favorite of Hume’s works, the Dialogues). He is showing his reader how his naturalistic philosophical project is itself rooted in human nature, and thus does not yield insight into the intrinsic structure of... (shrink)

What exactly is a genetic disease? For a phrase one hears on a daily basis, there has been surprisingly little analysis of the underlying concept. Medical doctors seem perfectly willing to admit that the etiology of disease is typically complex, with a great many factors interacting to bring about a given condition. On such a view, descriptions of diseases like cancer as genetic seem at best highly simplistic, and at worst philosophically indefensible. On the other hand, there is clearly some (...) practical value to be had by classifying diseases according to their predominant cause when this can be accomplished in a theoretically satisfactory manner. The question therefore becomes exactly how one should go about selecting a single causal factor among many to explain the presence of disease. When an attempt to defend such causal selection is made at all, the standard accounts offered (Koch's postulates, Hill's epidemiological criteria, manipulability) are all clearly inadequate. I propose, however, an epidemiological account of disease causation which walks the fine line between practical applicability and theoretical considerations of causal complexity and attempts to compromise between patientcentered and population-centered concepts of disease. The epidemiological account is the most basic framework consistent with our strongly held intuitions about the causal classification of disease, yet it avoids the difficulties encountered by its competitors. C.S. Lewis was a major public intellectual in Britain, beginning from the late 1930s and continuing to his death in 1963. In both his non-fiction, especially The Abolition of Man, and his fiction, most importantly in That Hideous Strength, he offers a critique of rationalism and scientism that is often strikingly similar to those that Michael Oakeshott penned in the late 1940s and early 1950s. This essay examines the question to what extent this similarity is merely superficial, and to what (...) extent the two writers shared a similar basis for their views. Furthermore, it examines whether there is an actual thread of influence: had Oakeshott been reading Lewis before writing his critique of rationalism, and if so, to what extent did Lewis’s work inform Oakeshott’s? Background: Good nurses show concern for patients by caring for them effectively and attentively to foster their well-being. However, nurses cannot be taught didactically to be “good” or any trait that characterizes a good nurse. Nurses’ self-awareness of their role traits warrants further study. Objectives: This study aimed (a) to develop a strategy to elicit nurses’ self-exploration of the importance of good nurse traits and (b) to explore any discrepancies between such role traits perceived by nurses as ideally and actually (...) important. Research design: For this mixed-method study, we used good nurse trait card play to trigger nurses’ reflections based on clinical practice. Nurse participants appraised the ideal and actual importance of each trait using a Q-sort grid. The gap between the perceived ideal and actual importance of each trait was examined quantitatively, while trait-related clinical experiences were analyzed qualitatively. Participants and research context: Participants were 35 in-service nurses (mean age = 31.6 years (range = 23–49 years); 10.1 years of nursing experience (range = 1.5–20 years)) recruited from a teaching hospital in Taiwan. Ethical considerations: The study was approved by the Institutional Review Board of the study site. Findings: Good nurse trait card play with a Q-sort grid served as an icebreaker to help nurse participants talk about their experiences as embodied in good quality nursing care. Nurses’ perceived role–trait discrepancies were divided into three categories: over-performed, least discrepant, and under-performed. The top over-performed trait was “obedience.” Discussion: Patients’ most valued traits (“patient,” “responsible,” “cautious,” and “considerate”) were perceived by participants as ideally important but were under-performed, perhaps due to experienced nurses’ loss of idealism. Conclusion: Good nurse trait card play with Q-sort grid elicited nurses’ self-dialogue and revealed evidence of the incongruity between nurses’ perceived ideal and actual importance of traits. L'émergence des TIC et deux changements politiques successifs ont amené une troisième vague de mouvements sociaux à Taïwan. Ces mouvements ont commencé à s'affranchir des partis politiques ; ils se préoccupent de problèmes sociaux et mobilisent différents modes de participation à la base. Cependant, les TIC ne peuvent à elles seules garantir leur succès. La plupart des études sur les nouveaux mouvements sociaux suggèrent que les TIC accroissent l'adhésion à ces mouvements parmi la population, ce qui facilite leur extension. Cette (...) étude analyse deux mouvements récents au sein de la société civile taïwanaise et conclut que ni la théorie de la mobilisation des ressources ni la théorie des nouveaux mouvements sociaux ne montrent comment les TIC seraient suffisantes pour structurer les mouvements de la société civile. En l'absence d'organisation concrète et réelle de la société civile, la participation purement virtuelle de la base risque d'en compromettre le leadership, la mobilisation, les prises de décision et les liens au niveau international.The emergence of ICT and two successive political changes have led to a third wave of social movements in Taiwan. These movements have begun to overcome the political parties they are concerned about social issues and mobilizing different modes of grassroots participation. However, ICT alone can not guarantee success. Most studies on new social movements suggest that ICTs increase adherence to these movements among the population, which facilitates their extension. This study analyzes two recent movements in civil society Taiwanese and concludes that neither the theory nor the resource mobilization theory of new social movements did not show how ICT would be sufficient to organize the movement of civil society. In the absence of concrete and actual organization of civil society participation purely virtual database could compromise leadership, advocacy, decision-making and international links. (shrink)

Recent studies from several different laboratories have provided further insight into structure-function relationships of cyclic nucleotide-gated channel and in particular the cCMPgated channel of rod photoreceptors. Site-directed mutagenesis and rod-olfactory chimeria constructs have defined important amino acids and peptide segments of the channel that are important in ion blockage, ligand specificity, and gating properties. Molecular cloning studies have indicated that cyclic nucleotide-gated channels consist of two subunits that are required to reproduce the properties of the native channels. Biochemical analysis of (...) the cGMP-gated channel of rodcells have indicated that the 240 kDa protein that co-purifies with the 63 kDa channel subunit contains both the previously cloned second subunit of the channel and a glutamic acid-rich protein. The regulatory properties of the cGMP-gated channel from rod cells has also been studied in more detail. Studies indicate that the beta subunit of the cGMP-gated channel of rod cells contains the binding site for calmodulin. Interaction of calmodulin with the channel alters the apparent affinity of the channel for cGMP in all in vitro systems that have been studied. The significance of these recent studies are discussed in relation to the commentaries on the target article. (shrink)
